Filling out a form for our CFH programs is a simple and straightforward process. The form can be accessed online or in person.
To begin, you'll need to provide some basic information about yourself, including your name, contact information, and relationship to the fallen hero. If you're a caregiver or foster child, you'll need to provide information about your current placement and any relevant background information.
The form will also ask for some information about the fallen hero, including their name, branch of service, and date of passing. If you're a foster child, you'll need to provide information about your previous placement and the circumstances that led to your placement.
Once you've provided the required information, you'll be asked to indicate which CFH programs you're interested in. You may select all the programs that apply to your situation.
After completing the form, you'll need to submit it to our program office. Our team will review your application and contact you within a reasonable timeframe to discuss your eligibility and next steps.
